Was definitely the last time it has happened in the VFL/AFL.
From what I can kind of recall, after it happened, it was said to have only happened around half a dozen times in VFL/AFL history.
Round 12 1924, another dirty day against Carlscum. One of the all time great acts of captaincy. The late, great Wels Eicke called for a head count.
After lining all the opposing players up and conducting the count, the umpire went up to Wels to inform him that both sides had the regualtion 18 players on the field.
Wels replied, "You forgot to count yourself," turned and walked away.

There have been only three head counts, all unsuccessful, in the history of the VFL/AFL:
by St Kilda captain Wels Eicke against Carlton Round 12, 1924;
by Essendon captain Jack Clarke against North Melbourne in Round 17, 1958;
by West Coast captain Guy McKenna against St Kilda in Round 22, 1999.[8]
